Dallas Family Practice, LLC, is pleased to welcome Glendon E. Cook, M.D. to its practice of Family Medicine. Dr. Cook is a native of Plymouth and a graduate of Wyoming Valley West Schools. He obtained a Bachelor of Science degree from Wilkes University and went on to earn his Doctor of Medicine degree from Drexel University College of Medicine in Philadelphia.
Dr. Cook is board eligible in Family Medicine with the American Board of Family Medicine. He is a graduate of the Wyoming Valley Family Practice Residency Program in Kingston where he served as Chief Resident.
He is a member of the Pennsylvania Academy of Family Physicians and the American Academy of Family Physicians. Dr. Cook resides in the Back Mountain with his wife and two children.

On April 14, 2003, the Health Information Portability and Accountability Act of 1996 (HIPAA) went into effect. Developed by the Department of Health and Human Services (HHS), these new standards provide patients with access to their medical records and more control over how their personal health information is used and disclosed. They represent a uniform, federal floor of privacy protections for consumers across the country. Dallas Family Practice, LLC is an independent, privately owned primary care medical practice. We serve patients of all ages and their families ranging from newborn to seniors.
Our physicians are board certified in Family Medicine and work in conjunction with our entire staff, including our certified Physician Assistant. We provide timely, quality medical care and we accept most major insurances.
